The following form is for CC students. Classes start on Oct 7th and will be held every Monday at 2pm. $90 per child per quarter (6 classes). 

October 7th-November 18th: Cooking Class
January 6th-February 17th: Craft Class
February 24th- April 14th: Cooking Class

Payment is due the week before each quarrter.

By completing and submitting this form I give permission for The Steadfast Project to photograph and/or record my child, with my full understanding and agreement that such still photographs, videos and/or screen/sound recordings, separately or in combination can be used in publications/broadcasts deemed necessary by said organization for publicity/promotion of The Steadfast Project. Further, I hereby release all rights, title and interest of said materials to the ownership of The Steadfast Project.
