The following form is for CC students. Classes start on January 22nd and will be held every monday at 2pm. $165 per child (11 total classes). 

By completing and submitting this form I give permission for The Steadfast Project to photograph and/or record my child, with my full understanding and agreement that such still photographs, videos and/or screen/sound recordings, separately or in combination can be used in publications/broadcasts deemed necessary by said organization for publicity/promotion of The Steadfast Project. Further, I hereby release all rights, title and interest of said materials to the ownership of The Steadfast Project.
